New Student Information Check  List

Submit the following information to the Counseling Department upon enrolling at Creek Wood High School.

____The contract person, school address, telephone number and fax number of previous school

____Birth certificate and social security card

____A transcript (unofficial or official) and/or report card from previous school

____A withdrawal form indicating the courses taken up to the date of withdrawal, number of absences and the grade received (if applicable) discipline record

____Verification of new address

____If student is living with a guardian, legal documentation must be presented

____If student is coming from a home schooling situation, any standardized testing information (will need to take placement test before enrolling)

____If student has an individual educational plan, please have a copy for enrollment purposes (confidential records)

____If student has a 504 Plan, please have a copy for enrollment purposes   

            (confidential records)

____Complete the request for records form

____Complete enrollment form

____ Tennessee Immunization Certificate. If transferring from out of state, carry your out of state immunization record to the Dickson County Health Department, 301 West End Avenue, Dickson, 446-2839. They will transfer the information to a TN card
