New Student Information Checklist

Please submit the following information to the Counseling Department upon enrolling at Creek Wood High School.

____Proof of Residence

____Proof of Guardianship

____Birth certificate and social security card

____ Tennessee Immunization Record must have proof of MMR-second dose

 If transferring from out of state, carry your out of state immunization record to the Dickson County Health Department, 301 West End Avenue, Dickson, 446-2839. They will transfer the information to a TN card

____Last Report Card for 9th grade student or Transcript (unofficial/official) for 10-12th grade

____Discipline Record from previous school

____Attendance Record from previous school

____Withdrawal form-if transferring after the school year has begun

____The contract person, school address, telephone number and fax number of previous school

____If student is coming from a home schooling situation, any standardized testing information (will need to take placement test before enrolling)

____If student has an IEP (Individual Education Plan), please have a copy for enrollment purposes (confidential records)

____If student has a 504 Plan, please have a copy for enrollment purposes (confidential records)
