YOUTH RISK BEHAVIOR INFORMATION NOTICE

 

    Our school is taking part in the Youth Risk Behavior Survey sponsored by the Tennessee Department of Education, East Tennessee State University, and the Centers for Disease Control and Prevention. Students in Middle School and High School have been selected to participate in this survey. The survey examines elements of youth health related behaviors. Survey areas include tobacco use, alcohol and drug attributes and behavior, adolescent sexuality, personal safety and physical activity levels.

    A required statement- Doing this paper and stencil survey will cause little to no risk to your child. The survey has been designed to protect your child’s privacy. Students will not put their names on the survey or be identified in any way. Individual schools and/or school districts will not be mentioned by name in reports from the results of this survey. Your child will derive no immediate benefit from participating in this survey. Survey results will be invaluable in addressing the exact needs of our students. The results of this survey will enhance Tennessee’s ability to design, implement and evaluate the youth component of the Coordinated School Health Initiative.

    Participation in this survey is completely voluntary. In order for your child to be considered for participation in this survey, you need do nothing. However, if you do not wish for your child to participate in this survey you must return a Denial Form.
